VR Table Tennis Scores 79 Out of 100 With Older Adults Facing Memory Decline
A new mixed methods study tested a virtual reality table tennis game with 12 older adults living with mild cognitive impairment. The game earned a usability score of 79.38 on the System Usability Scale, well above the acceptability threshold, and participants could play independently after one supervised session. The findings point to table tennis as a leading candidate for digital brain-health interventions.

Older adults living with mild cognitive impairment can play a virtual reality table tennis game independently, and they rate it highly, according to a study published in JMIR Serious Games in August 2026. On the System Usability Scale, a standard 0-100 benchmark where anything above 68 counts as acceptable, the game scored a mean of 79.38 (SD 12.89, 95% CI 71.19-87.57) among a dozen players aged 60 and over.
The number matters because of the problem it targets. Mild cognitive impairment, the stage between normal aging and dementia, affects roughly 15 percent of people over 65, and physical activity is one of the few interventions clinicians can actually prescribe for it. The catch is adherence. Older adults with memory and attention difficulties are among the least likely to start an exercise routine, and among the quickest to drop one.
Why the Researchers Chose Table Tennis
A team led by Xiangya School of Nursing at Central South University in Changsha, China, built the game through a structured, user-centered design process. Group discussions with older adults and a multidisciplinary panel of eight experts compared candidate activities and hardware platforms. Table tennis won on two grounds: visual clarity and physical comfort. Participants also knew the sport. In China, where the study took place, table tennis is culturally familiar, which lowered the learning curve before a single headset went on.
The final prototype, whose name translates roughly to “lively table tennis,” runs on a standalone PICO 4 Pro headset and was built in Unity3D. Players swing a controller to return balls, with speed and difficulty tuned for people who may have slowed reactions and limited mobility.
The choice is consistent with the wider evidence on the real-world sport. A 2026 meta-analysis of 14 randomized controlled trials covering 1,565 participants found table tennis interventions produced large gains in both balance (standardized mean difference 0.78) and cognition (SMD 2.05) in older adults (Healthcare, 2026). A game that simulates the sport cannot automatically claim those effects, but it inherits a strong rationale.
How the Testing Worked
The evaluation phase recruited 12 older adults with mild cognitive impairment from community, home-based, and institutional care settings in Changsha. Their mean age was 69.08 years (SD 6.08). Each participant played a supervised, independent gaming session, then completed the System Usability Scale, a ten-item questionnaire that is the most widely used usability benchmark in health technology research. Face-to-face semistructured interviews followed, and the team kept interviewing until data saturation, which arrived at the ninth interview; all 12 completed the study.
The mixed methods design let the researchers check whether what people said matched what they scored. It largely did. Quantitative usability and learnability indicators converged with interview themes praising simple operation and built-in safety functions.
Where the Honest Friction Showed Up
The interviews surfaced two main themes and seven subthemes, and not all of them were positive. Learning friction came from controller button complexity and general unfamiliarity with VR. Some users found on-screen buttons too small.
The most consequential finding was a gap between confidence and intent. Participants felt they could use the game, yet their willingness to play it frequently over the long term was only moderate. The interviews explained why: physical discomfort, including VR dizziness and facial pressure from the headset.
This is the paradox the authors highlight. A game can score well on a usability questionnaire and still carry comfort barriers that determine whether anyone plays it in week six. For a population that struggles with adherence in the first place, dizziness is not a footnote. It is the ballgame.
What This Adds to the VR Rehabilitation Picture
This is not the first time virtual table tennis has been tested in a neurology-adjacent population, and the pattern building across studies is notable. A 2026 pilot randomized controlled trial from a Japanese group used a commercial VR table tennis program with 10 Parkinson disease patients and reported no serious adverse events (Neurologia i Neurochirurgia Polska, 2026). The new Chinese study extends the approach to mild cognitive impairment with a game designed from scratch around its users rather than adapted from hardware aimed at younger players.
The design framework itself may be the most reusable output. The authors distilled their process into an evidence-based template for building cognitive-impairment-specific exergames, which other teams can copy.
The Limits, Stated Plainly
This was a single-group usability study with 12 participants, not a randomized trial. It measured whether people could use the game and how they felt about it. It did not measure whether playing improved memory, attention, or any other cognitive outcome. Those trials are the logical next step, and until they exist, no one can claim this game treats or prevents anything.
The usability bar, though, is a real prerequisite. Digital exercise interventions for brain health have failed for decades on this exact step, with interfaces that confuse the very people they aim to help. A culturally familiar sport, a headset chosen for comfort, and a 79-point usability score from players with mild cognitive impairment is a genuine proof of concept for the pipeline, whatever the cognitive trials eventually show.
For the roughly one in seven older adults living with mild cognitive impairment, the near-term takeaway is simpler. The activity most worth defending in recreation centers and basements is the same one now being rebuilt for headsets: table tennis, at any speed, on any table.
