Table Tennis Beats Conventional Vestibular Exercises for Balance Recovery

For millions of people with balance disorders, conventional vestibular rehabilitation often feels repetitive and boring. A 6-week randomized trial shows adding table tennis to standard exercises significantly improves vestibular function and postural control compared to traditional exercises alone.
The Study Design
Researchers at Istanbul Medipol University recruited 72 healthy young adults aged 18-21 years. Participants were randomly assigned to two groups:
- Conventional Vestibular Exercises (CVE) group: 36 participants performed standard vestibular rehabilitation exercises twice weekly for 6 weeks.
- CVE-TT group: 36 participants performed the same conventional exercises combined with table tennis exercises.
Both groups trained three phases over six weeks, with sessions twice per week. The CVE-TT group used table tennis rackets, balls, and balance equipment to challenge their vestibular system through dynamic sport movements.
Measuring Balance
Postural stability was measured using Computerized Dynamic Posturography (CDP) with the Sensory Organization Test (SOT). This gold-standard test evaluates how individuals use three sensory systems to maintain balance:
- Somatosensory: feeling body position through touch and pressure
- Visual: seeing the environment
- Vestibular: sensing head movement and orientation through the inner ear
Participants completed six conditions that progressively removed or distorted sensory inputs. For example, one condition swayed the visual surround to confuse the eyes while another moved the platform to destabilize somatosensory feedback.
Results - able Tennis Wins
The CVE-TT group showed statistically significant improvements across multiple balance metrics:
- Strategy 5 and 6 scores - ignificant improvement after training (p<0.05, p<0.01)
- Composite balance score - ignificant improvement (p<0.05)
- Vestibular ratio score - ignificant improvement (p<0.05)
Strategy 5 and 6 conditions are the most challenging tests that minimize somatosensory input and remove visual information, forcing reliance on the vestibular system. Improvements here indicate better vestibular function.
In contrast, the CVE-only group showed only minimal improvement: a significant increase only in Strategy 6 and preference scores, with no significant change in vestibular or composite scores.
Reduced Visual Dependence
Perhaps the most clinically significant finding: the CVE group showed increased dependence on visual information after training, while the CVE-TT group decreased visual dependence.
Visual preference indicates how much someone relies on eyesight for balance rather than vestibular or somatosensory systems. When vestibular function is weak, people over-rely on vision—which explains why many balance disorder patients feel dizzy in dark environments or on uneven surfaces.
By reducing visual dependence, table tennis training helped participants trust their vestibular system more.
Why Table Tennis Works
Table tennis provides several unique benefits for vestibular rehabilitation:
Dynamic head movements - layers constantly track the ball with their eyes while moving their head and body. This stimulates the vestibular-ocular reflex—the connection between eye movement and head position sensing.
Rapid weight shifts - ateral footwork and quick stance changes challenge the vestibular system to maintain balance during sudden movements.
Dual-task demands - layers must balance physically while making quick strategic decisions. This cognitive-motor integration mirrors real-world balance challenges.
Fun and engaging - nlike repetitive clinical exercises, table tennis is enjoyable. This improves adherence to rehabilitation programs.
Practical Applications
The researchers note several important implications:
- Home-based rehabilitation - able tennis can be done at home with minimal equipment, reducing the need for clinic visits.
- Cost-effective - racket, ball, and table cost far less than specialized balance training equipment.
- Scalable - rograms can be adapted for different skill levels, from wall rallies to competitive play.
- Entertaining - atients are more likely to stick with a program that feels like a game rather than exercise.
The study suggests this approach may be particularly beneficial for people with residual dizziness or persistent postural perceptual dizziness who feel unbalanced despite normal clinical test results.
Study Limitations
This was a preliminary study with healthy young adults. The authors acknowledge the need for future research in older adults and clinical populations, particularly those with:
- Benign paroxysmal positional vertigo (BPPV)
- Chronic unilateral peripheral vestibular disorders
- Persistent postural perceptual dizziness (PPPD)
- Residual dizziness after vestibular injury
However, the fact that significant improvements occurred in healthy participants whose balance scores were already normal suggests even stronger effects may be possible in individuals with actual balance impairments.

For people recovering from vestibular disorders, adding table tennis to conventional rehabilitation exercises may significantly improve outcomes. The sport’s unique combination of dynamic movement, visual tracking, and cognitive demands challenges the vestibular system in ways traditional exercises cannot.
This 6-week trial demonstrates that balance rehabilitation does not need to be boring or clinical. Sometimes, the most effective therapy is a game.
